Key Data and Facts
Full rate for self-employed: 14.6% + supplementary rate (no employer share). Minimum assessment base 2026: 1,248.33 euros/month. Long-term care: 3.6% (or 4.2% if childless).
Step-by-Step Guide
How to calculate the contribution to voluntary statutory health insurance (GKV) step by step: 1. Determine total income: for voluntarily insured persons all types of income count -- profit from self-employment, capital gains, rental income, pensions. 2. Note the minimum assessment basis: in 2026 it is 1,248.33 EUR/month. Even with lower income, at least this amount is used as the basis. 3. Maximum assessment basis: 5,812.50 EUR/month (69,750 EUR/year). 4. Apply the contribution rate: self-employed without sick pay: 14.0 % + supplementary contribution (approx. 2.5 %). Self-employed with sick pay: 14.6 % + supplementary contribution. The full contribution is borne alone (no employer share). 5. Add long-term care insurance: 3.6 % (with children) or 4.2 % (childless from age 23). 6. Calculate the total monthly contribution. 7. Check the GSVG subsidy: voluntarily insured self-employed persons may be able to apply for a subsidy at the responsible institution.
Calculation Example
Self-employed, profit 3,500 EUR/month, childless, without sick pay: health insurance = 3,500 x (14.0 % + 2.9 %) = 591.50 EUR. Long-term care (4.2 %): 147 EUR. Total contribution: 738.50 EUR/month.
